•	Ask your healthcare provider if Alpha-Stim® is appropriate for you. All rentals of Alpha-Stim® units require a prescription (Rx) or a statement of medical necessity.
	Download a Statement of Medical Necessity Form for Alpha-Stim® purchase or rental
•	Request and fill out one of our Alpha-Stim® rental agreement forms.
	Contact us to obtain a Rent-to-Own Agreement Form for Alpha-Stim ® 100, SCS or PPM
•	Fax your rental agreement and Rx to 888-684-8414. If you prefer, you may scan both documents and send them to us at info@alleviahealth.com as email attachments, or you may mail them to: Allevia Health, Inc. 2312 NW Kings Blvd. Corvallis, OR 97330.
•	Once we receive your Rx and a signed rental agreement, an Alpha-Stim® unit will be mailed to you by 2nd Day Air to the address specified in the rental agreement.
•	We will be available by phone to help you learn how to use your Alpha-Stim® device. Your rental plan includes free technical support.
•	Your credit card will be billed a low monthly fee for each month of rental.
•	You may terminate the rental agreement by returning the unit at any time or you may continue paying monthly rental fees until the balance is fully paid off. All rental expenses count towards the purchase price of your own device.
•	You may purchase your unit at any time by paying off the remaining balance.
