Filing
We
offer insurance filing services for the Alpha-Stim® 100 when
purchased directly from Allevia Health, Inc. Most private insurance
plans approve the Alpha-Stim® 100 when this device is prescribed
for some type of chronic
pain in the body (below the head). A partial payment of less
than 50% of the retail price of the device is typical. Depending
on the allowable amount, co-insurance, and out-of-network benefits,
the payment may be well below 50% and the approved amount may
be applied to the client's deductible. The Alpha-Stim® is
always sold out-of-network; there are no in-network distributors
(preferred DME providers) for this device
at this time.
For
diagnoses other than pain below the head, and for the Alpha-Stim® SCS,
we do not offer insurance filing at this time.
We
regret that we are not able to file with Medicare/Medicaid
or with
private plans associated with Medicare at this time. We
do file claims with Tricare. For the VA, or if you have an
active motor
vehicle claim, please contact us for more information.

of Medical Necessity (or prescription), and
(c) Rent-to-Own form to us at (888)
684-8414 or mail all forms to Allevia Health,
Inc. at the address below. We will ship your Alpha-Stim® to
you immediately and offer follow-up support. In this case
there is no fee for filing a claim with your insurance.

If
you prefer that we file with your insurance first, prior
to renting or purchasing the device, there will be a $50
non-refundable filing fee payable prior to filing a claim;
we can check your benefits at no charge but please note that any
information we receive during pre-authorization of your claim
is not
binding
to
the insurance company and the end result may be different
from
verbal quotes
provided
by
insurance case managers.
Under no circumstances can we guarantee a positive
claim decision
or any particular amount of payment.
STEP
3: It usually takes 30-60 days for an insurance company
to fully process a claim for an Alpha-Stim® device. If
your insurance claim is approved, the payment we receive from
your
insurance company will be applied to the purchase of your Alpha-Stim® or
to the latter payments on your rent-to-own plan, thus eleminating
those payments. If your insurance company pays the full retail
price of your Alpha-Stim®, which is very unlikely unless
you have an active motor vehicle claim, then we will refund
your rental payments, including the damage deposit, or the
purchase amount entirely.
What if my insurance does not cover the Alpha-Stim®?
For
most patients the Alpha-Stim® is partly, or entirely, an
out-of-pocket expense. Clients with flexible spending accounts
(medical FSA,
HSA, HRA) may claim the cost of their device and supplies as
qualified medical expenses. We also offer
different rent-to-own options
on all three Alpha-Stim® models
to meet your needs. With our rental plans you may rent a new
Alpha-Stim® device for as little as $85 - $100/month
(see the rental contract for details). You may return your rental
unit at any time,
with
no
further
payments due, or if you continue
renting, you
will ultimately own your device at the end of the rental
period. Please keep in mind that even if you pay for your Alpha-Stim® out-of-pocket,
given its efficacy and safety, this technology is one of the
most cost effective treatment options available for the treatment
of anxiety,
depression,
insomnia
and
pain. It also comes with five year warranty and technical support
at no charge.
